Sign-Up Agreement Language
The following disclosure is presented to end users at the point of opt-in across all consent methods:
"Naturalstaterecovery SMS — iCampaign Alerts. By opting in, you consent to receive recurring automated SMS text messages from Naturalstaterecovery to the mobile number provided. These messages may include follow-ups, reminders, application updates, and supportive resources related to behavioral health services. Consent is not a condition of purchase. Message frequency varies. Message and data rates may apply. Reply STOP to cancel at any time. Reply HELP for help."
Campaign Registration
Naturalstaterecovery sends SMS messages to individuals who have expressed interest in behavioral health and recovery support services. Messages are used to provide follow-ups related to recovery program inquiries, reminders, application updates, and supportive resources related to ongoing behavioral health communication previously initiated via email, phone, or web forms.

- Naturalstaterecovery: You began your sunrise recovery application recently. Would you like help completing the process? Naturalstaterecovery is here to support you. Reply STOP to opt out or HELP for options.
- Naturalstaterecovery: We wanted to follow up regarding your interest in recovery support services. If you have questions or would like assistance, Naturalstaterecovery is here to help. Reply STOP to opt out or HELP for options.
- Naturalstaterecovery: Just checking in about your recovery program inquiry. Naturalstaterecovery's team is available if you need guidance or updates. Reply STOP to opt out or HELP for options.
Opt-In Process
End users can opt in to receive SMS messages through the following compliant methods. All methods are optional — consent is never a condition of purchase or service.
1. Phone / CRM Entry (Verbal Consent)
During an intake or call with a representative, the user may provide verbal consent to receive SMS messages. Representatives are required to clearly state the following verbiage verbatim:
"By providing your phone number, you agree to receive recurring automated SMS text messages from Naturalstaterecovery (iCampaign Alerts) related to behavioral health follow-ups, reminders, and recovery support updates. Message frequency varies. Message and data rates may apply. Consent is not a condition of purchase. You may reply STOP at any time to cancel, or HELP for assistance. Do you consent?"
The representative must record the subscriber's verbal confirmation ("yes" / "I consent") in the CRM along with a timestamp and the rep's ID.
2. Email Link or Web Form
Users opt in by clicking a preference link included in every outgoing email, which leads to the web-based communication preference center. The SMS checkbox is unchecked by default — users must actively check it to subscribe. The form displays the following disclosure adjacent to the SMS checkbox:
"By checking this box, you consent to receive recurring automated SMS text messages from Naturalstaterecovery (iCampaign Alerts) to the mobile number on file. Consent is not a condition of purchase. Message frequency varies. Message and data rates may apply. Reply STOP to cancel. Reply HELP for help."
3. Keyword-Based Opt-In (START)
Users may initiate opt-in by texting START to Naturalstaterecovery's designated phone number. Upon receipt of this keyword, the system records their consent and immediately sends a confirmation message:
"Naturalstaterecovery (iCampaign Alerts): You are now subscribed to receive SMS updates. Message frequency varies. Msg & data rates may apply. Reply STOP to opt out or HELP for help."
4. Signed Document / Paper Form
Users may provide written consent by signing a physical or electronic intake form that includes an SMS opt-in clause. The clause must appear as a clearly labeled, standalone checkbox or signature field — separate from any other terms — and must not be pre-checked or pre-filled. The required disclosure language on the form is:
"By signing below / checking this box, I consent to receive recurring automated SMS text messages from Naturalstaterecovery (iCampaign Alerts) at the mobile number provided on this form. These messages may include follow-ups, reminders, and recovery support updates. Consent is not a condition of receiving services. Message frequency varies. Message and data rates may apply. Reply STOP to cancel at any time. Reply HELP for help."
Signed forms are scanned and stored in the CRM linked to the subscriber's record. The rep entering the subscriber must mark the SMS opt-in field in the CRM and note "Paper form" as the consent method along with the date the form was signed.
Opt-Out Process
Users may opt out at any time using any of the following methods. Opt-out requests are processed immediately.
1. Keyword-Based Opt-Out
Users can text any of the following keywords to any of Naturalstaterecovery's sending numbers: STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT, or REVOKE. The system automatically processes the request and confirms via SMS:
"You have been unsubscribed from Naturalstaterecovery SMS updates. No more messages will be sent. To resubscribe, reply START."
2. CRM Update (Manual Opt-Out)
A support representative may mark a subscriber as opted out if the user verbally requests to stop receiving messages. This must be documented in the CRM with a timestamp and the representative's ID. No further SMS messages will be sent after the opt-out is recorded.
3. Email Preferences / Web Form
Users may manage their SMS preferences through the communication preference center linked in every email. Unchecking the SMS checkbox and saving the form is treated as an opt-out request. Naturalstaterecovery's campaign system includes an unsubscribe link in every email, and each SMS campaign is linked to a corresponding email message.
Evidence
All opt-in and opt-out events are logged in the CRM with a timestamp, the consent method used, and the identity of the user or representative who recorded the action. This evidence is retained for compliance and audit purposes.
